    Advance Health is focused on improving the quality of life for health plan members through health assessments, effective care coordination and best in class care management. The Advance Health Care Management Program is for health plan members residing in their own home, long-term care, residential care, or other  facilities.

    Advance Health is seeking a Clinical RN Team Lead with management experience, expertise with complex populations, experience working in multidisciplinary teams, and the desire to make a difference in people's lives. While there are many goals of this program, the overriding one is to improve the member's quality of life and health outcomes.

    As the Clinical Team Lead, you will oversee the day-to-day clinical operations and have an opportunity to work with a multidisciplinary team. We're seeking an energetic candidate that possesses excellent leadership skills, innovation, and a passion for excellence in exceeding the goals of the populations we serve as well as our clients.


    • Leading the team conducting primarily telephonic assessments and ongoing care coordination with members and/or caregivers, as well as appropriate medical providers based on care needs. Ensuring high quality team performance to ensure we meet our clients' expectations in terms of quality and production within prescribed
    • Assist with day to day operations and coverage with telephonic assessments when staffing requires.
    • Continuously evaluates, reports and implements improvements to impact staff productivity and quality of performance through available telephonic and electronic medical records, and supportive data analysis and outcomes reporting
    • Improves team's strategies around locating, engaging, assessing and managing assigned members
    • Conducts team meetings to update staff on best practices, process changes, and continuing expectations when needed
    • Guarantees that the team members have the necessary education and training/retraining to effectively participate on the team
    • Provides quality customer service, including interacting with Advance Health team, Clients, members, and Care Management team
    • Ensures that the team addresses all relevant issues surrounding Care Coordination efforts within the specifications and standards outlined in training
    • Continually assists in the creation of processes that assist in the overall goals of the Care Management team
    • Collaborating with management team to continue to make programmatic improvements and to help grow Advance Health's Care Management program
    • Partnering with the Advance Health Care Management team to quickly respond to urgent issues
    • Leading weekly rounding with the Advance Health Care Management team
    • Assists with onboarding, orientation, and evaluating team
    • other responsibilities as assigned


    • Bachelor's Degree in Nursing preferred
    • Managed Care, Transitional Care or Case Management experience required appropriately
    • Strong assessment skills to recognize urgent/emergent situations and respond appropriately
    • Collaborative and team-oriented approach to healthcare delivery services
    • Ability to tightly manage to prescribed deadlines and quality  standards
    • Excellent communication skills. Ability to present to internal leadership team and to client
    • Excellent critical thinking and problem solving skills
    • Strong organizational skills
    • Prompt, responsive, and flexible with a passion for patient care
    • Experience working with culturally diverse populations
    • Ability to work in a fast-paced environment
    • Ability to obtain unrestricted licensure in required states
    • PC literate with working knowledge of Microsoft Office and various computer software programs
    • Ability to shadow team members and/or conduct home visits using own vehicle (mileage is reimbursed), as needed/applicable; Valid driver's license and proof of insurance required
    • Position based in the Richmond office
