This position is responsible for program support and direct member interaction to help coordinate Readmission Prevention Program/Care Management visits. Process inbound and outbound calls. Handle all member cancellations, schedule/reschedule requests and reminders for upcoming appointments. This role also involves ongoing analysis of data from a variety of sources to continuously identify, recommend, and help implement operational improvements. This role involves attention to detail, robust organizational skills, and involves interaction with Clients, the Care Management team, providers, and members. It also requires proficiency with analytical tools (Outlook, Excel, etc.), strong communication skills and a strong commitment to and affinity for delivering the highest level of customer service.
